Certificate LoR with non-specified Specialty

As per AAMC instructions:

The Letter Request Form is a PDF that includes the following:

LoR Author Details (as you entered them)
LoR Author Name
LoR Author Title/Department
Applicant Details
Name
Contact InformationERAS 2014 - MyERAS User Guide for Residency
ERAS Letter ID
AAMC ID
Specialty to which this letter will be assigned

Does the last option mean that if we use letter request form, we can't use that LOR for multiple specialities?

You can choose (non-specific) from the drop-down menu and that way you leave it general and can use it for multiple specialties.

However, I have to remind you that when your letter is not specific for the specialty it will carry much less weight compared to a specific specialty LoR.

The ObGyn Letter would not be effective in applying for General Surgery. For the purpose of just completing the profile (avoiding the filter) yeah it might work but once they read it they'll immediately drop you out.
General surgery is very competitive and they'll take every opportunity to drop applicants until they end up with only elite applicants to send them interviews.

If you submit 2 GS LoRs only, you will be filtered immediately.
If you submit 2 GS LoRs + 1 another LoR, you will be filtered later on.
So both options will end you up with nothing.
The only solution is to get a 3rd GS LoR.

Would it hurt to put in pediatrics endocrinologist LOR alongwith 2 other IM endocrinologists LOR's for IM application, given that I want to go for endo at the end of the day?

Yeah that would work but make sure that the pediatric endocrinologist recommends you for IM residency not Peds residency and specify the specialty as IM not peds.

I have listed 3 LOR and specified Internal medicine and neurology. Would that be okay? I will be getting 1 from neurologist, 2 from IM. Let me know if thats okay? Or should I change the speciality to purely IM and Neuro.

If I am applying for IM and pediatric, how should I ask my professor to write LOR? Mentioning just ped/Im or both can be ok?
