Question Is this how you finalize LOR authors?

I know this maybe simple but can somebody confirm please.

LoR Author Name: John Smith
LoR Author Title / Department: MD/Surgery
Specialty to which this letter will be assigned: Internal Medicine

Yes.

But you might want to replace 'MD' with 'professor'/'assistant prof' etc

Also, don't use a surgery letter for IM.

Thanks for responding. The doctor I'm getting the letter from is not a professor or an assistant prof. He's just an 'MD'

In that case I would use 'General Surgeon/Hematologist/Cardiologist' etc instead of MD.
