Mail How to write email reply to IV invitation

Ok I have a question,I know it's going to sound a LIL stupid but I am just curious...Wat are the components of an email response to confirm the iv..
I should look professional, so after confirming what do we have to write our designation...eg
Poonam aslam MD
Post doc fellow
Dept of ......
University of ......
Phone...or how shud it end or start??

Maybe something like....

Name of the person****
Position eg. Program Coordinator
Name of the program maybe??

I am pleased to receive an invitation for a residency interview at ***** program.

I would like to schedule the interview for *date*.

I look forward to meeting you.
