Default How to ask for LOR at the end of elective?

I am doing a 4 week long elective at a university in US. Its been 2 weeks already and have worked very hard. I am wondering how to ask for a lor.

what should i do about waiver? If I waive the rights to see the LOR, I am fearing that my attending may not be able to remember me properly in september when i will be applying for the match. Shall I do that or ask for a unwaived one ?

Also in the whole rotation i have been mainly in touch with one attending and the elective coordinator of the department just knows me by face. Shall i ask both of them?

Be honest, tell him/her , you are planning to apply next match, an you wanted to ask him for a LOR. Waiver is always better, it is the way all the AMG do it.

The correct way is to ask you attending for an appointment at the end / near the end of the rotation:

"Hey Dr. XY, thank you so much it has been a great time, I learned so much.. etc. I was wondering if you could take a few minutes of your time some time next week to discuss my career plans?"

Attending: Sure, why don't you send me an email..

"Dear Dr. X. thank you so much I just wanted to ask when would be a good time to meet next week. Thank you very much, sachinkumar"

time passes:

On appointment day: "Thank you for taking the time. I am planning to apply next season and just wanted to ask about advice for my career." Ask questions... Then: You have worked with me and given me a very high evaluation. As someone who is familiar with the quality of my work, would you feel comfortable to write a strong letter of recommendation for me?

Then you send all the relevant stuff you did during the rotation, and during your medical ever, to give your attending the chance to write a stellar LOR.

e.g. Thank you so much Dr. XY for your advice and for writing a letter of recommendation for me. Here is some information to help you write the letter.

e.g. CV, presentation slides, list of publications, awards, anything that makes you look good.

This is the ideal way to ask for a letter. Always waive your right to read it. Good luck !

Okay. Thank you so much for the reply. What shall I do to make my attending remember me till august when he will upload the letter. I am leaving USA in Feb and I really want him to remember me and write a proper letter and not a generalized one

ask him to write + file the letter now and then upload it when you have your ERAS token etc. (it usually takes the 1-2 months to write, anyway)

What is exactly meant by "file it now"? Does this mean store the LOR in a safe place and upload it to ERAS when required?
