I am not sure if that is possible, the traditional fellowships that FM are elegible for, Geriatrics, and if they are lucky, Pain, Sport, or Adolescent. But they usually give preference to PMR and Pediatric residents. There are some family physicians. With preferred OBgYN practice, but that is entirely up to the group or the hospital.

Are you a USIMG or a non-US IMG? 199 is a really low score for Step 1 even for family med. You have to blow Step 2 out of the water and even then I'm not sure if there's any guarantee of an interview.

Also, you can do women's health fellowship after family med.

There are obs/womens health track fellowships that allow you to do ob/gyn. Meaning C Sections and any procedures based on competency/malpractice coverage. Again as the poster above me has stated, that step score will really haunt you. First when you apply to FM, then when you apply for the fellowship.

Also, if you want to do everything in OB/Gyn ex. high risk, MFM etc. go for OB/Gyn. Not Fellowship trained Ob/Gyn.

For primary care sports medicine family medicine is given preference over any other specialty because of the fact that its basis requires you to have an outpatient practice facility where you will see everything from ob/gyn to peds to geriatrics.

Pain fellowships are available, but I can't comment on the competitiveness.
