Certificate What paper to print the LoR on?

Hi guys...
I'm new to this forum; I need some help with the LOR's; I am an IMG and I completed my step 1 and 2 and I am waiting for the ECFMG certificate; in the meantime I am doing an observership; at this moment I only have been promised one LOR from an attending physician; he asked me couple of days ago about it and how he can help me; i told him than i want to choose that option where I will not be able to see what he will write in that letter but now i am kind of confuse how he will be able to write it for me; i also know that he must write on that LoR my USMLE id or/and AAMC ID; if anybody can guide me how to do this, I will highly appreciate it;

1. Do I give him a simple paper where i type my USMLE id or he will do it when he will write it
2. Is there any specific format of the letter or is just a regular piece of paper
