Default Advanced Neurology Programs

As far as I know advanced program starts at PGY2 which means an applicant has to apply for a preliminary PGY1 internal medicine position simultaneously. Lets say I am applying to an advanced neurology program at Program X, so I need to apply for PGY1 IM at the same university? Or can I apply at some other place also?

And does it often happen that an applicant gets matched to the advanced program but does not get matched to the prelim IM position? Its kinda confusing.

I have the same doubts...
And what about LORs? All my LORs are specific for Neuro, do I need different ones to apply for preliminary IM?
