Hospital Applying to two programs in one University!

We can apply for more than 1 program in every university but if I apply for 2 programs in 1 university, won't they take that in such a way that I'm not sure which one to take and I don't have particular interest in 1 field, won't it lead to my rejection in both fields...?

If you apply to closely integrated programs in the same hospital such as neurology and internal medicine, then there is a chance that they know that you have applied for both and there's a chance that your application get rejected.

But if you apply to different specialties like internal medicine and surgery then it's unlikely that you will be caught.

We used to think that the PDs share information about applicants and that the GME office which in some institutions organize the interviews may highlight this issue to the PD, However with our ongoing experience we found that this is not true. PDs for example in IM and Neuro don't communicate and so neither will know what you are applying to except if the neurology program was an advanced one and you applied to both prelim and categ IM and you mentioned that you need PGY1 with them infront of the neuro PD. There are many IMG friendly IM programs insuch a way you can avoid applying to both programs in the same hospital most of the time.
