J1 Visa Programs requiring J-1 visa

Hi everyone..
There are some programs "requiring J-1" visa! I thought that in order to join a residency program they should sponsor that for you. So do they mean by requiring J-1 visa is that they are gonna sponsor it, or that they demand it on application?

It means that they require you to apply with the ECFMG for it and they'll accept it.

Info I don't get it!!

So nothing different from other programs that you'll apply to, and be sponsored by ECFMG because you are enrolled in their program. I don't really get it..
To get a J-1 visa you have to be accepted in a program, so that ECFMG will sponsor you. So how can I have the visa when I'm just applying for the program, i.e not accepted yet?

I don't get how sometimes programs require you to already have a J-1 visa while their sponsorship and acceptance is needed to get that kind of visa!!!

Totatlly agree ...i always ask myself this question ......Any answer???
