Default I need an experience based advice regarding my Visa issue

hello every one ,

I am in a very confusing situation regarding my Visa for which I think that every advice will be of value

first let me emphasize my current visa status and essential data so one can evaluate me on a realistic data

JOB- PGY2 ANESTHESIA RESIDENT in my home country

VISA -- J1 visa for elective accepted on NOV 2012 , but I didn't use it as it was issued after the date of my elective start.

step 1 pass 230 on june 2014

step 2 CK pending results ( ask Allah for me )

B1 Visa for 5 months of multi observer ships -- applied on sept.2014 and rejected on 212 B base , the officer said to me your financial situation is good and your carrier and CV also seems good but I cannot accept you on

B visa for 5 months .

I asked some programs to sponsor me on J1 VISA but all of them refuse that , Just B1 sponsor letter

I didn't apply for Visa again as I don't want to have multiple visa rejections on my passport and also I was preparing my self for step 2 ck

Now ,
what is the opinions for me

1- continue searching for a position in a non paid research or observer ship program that can sponsor me for J VISA

2- pay for CS exam and apply again for B1 visa -- in that choice the counselor for sure will ask me" hey man you are searching for any course to travel to USA , last year you was traveling for observer ships, now you are assuming going for an exam I think that you have an immigration intend --- "

He / she will never let me explain the total pathway , he will stamp the rejection on the paper giving me the blue paper and say just read it and it will answer your questions , the meeting had ended -- that is a very catastrophic famous scenario in American embassy in Cairo .

What shall I do now

please I wanna hear a realistic and experience based advices
