Passport VISA for USMLE CS

I applied for USMLE step 2 CS exam I received the scheduling permit and the ECFMG Visa letter , Is this enough to apply for a visa to take the CS exam ? Or do I have to select the test center and date before applying for the VISA?

Any help is highly appreciated.

Does get tougher. . . best is to get a J-1 and then work on other things.
