J1 Visa J1 visa without statement of need - is that possible???

Hey guys. Hope you are all fine.
Quick pre-history: I'm originally from post-USSR country with some "specific political direction" due to which it became physically impossible for me to obtain statement of need from ministry of health-care. From here, I have a quick question. Is that possible to get J1 without having statement of need? I mean, have you ever heard about this possibility? We all know requirements for ECFMG sponsored J1, but a lot of programs sponsor J1 by themselves. So, I'm wondering is that the rule of thumb for J1 in general or there is difference among different institutions for sponsoring J1?

Programs can't sponsor J1 by themselves if this is "alien physician J1". They could only accept sponsorship from ECFMG; even university-based programs.
