No on J1 you are wont be able to change status, you will need to be on H1b to change status. If you are applying for match next yr why dont you take the step 3 which is required for H1b.

Its best to contact an immigration lawyer for this purpose or your brother who is applying on your behalf. I am sure he is working with an immigration lawyer for your application. I dont think anyone on this forum will be able to give you details.

Well the US system has changed significantly, there are not many hospitals that offer H1B. This is how I would approach things and this is my opinion:

- It will be Ideal for you to get a residency in a hospital that sponsors H1b but not that high of a chance and plus you dont have step 3 yet. If you do match in a hospital like that I would initiate the process for J1 and talk to PD to allow you to take step 3 so you can come on H1b instead. Ideally and this is what I am planning to do, take step 3 around feb 25th. At this point rank list is locked and if you match at a hospital who sponsors H1b you are ready to go.

- Now there is a chance that you match at a hospital with J1, OFF COURSE you will take it. The biggest mistake you can make is not to a residency. Every year you go unmatched, there is less chances of you matching in general. Now with that scenerio you do your residency and apply for J1 wavier. Once you have the waiver you status is changeable.

- Now say you dont get a wavier and have to go back home. Its better to be a licensed doctor and go home for 2 yrs and come back then wait for a residency.

SO BASICALLY YOU WILL TAKE THE RESIDENCY SPOT. THAT IS YOUR PRIORITY AND NOT THE US IMMIGRATION STATUS.
