Default Need Help with Visa issues

I am in need of some guidance towards Visa issue that I am having for a research position.Actually I am getting a voluntary research position till June next year starting as early as possible but the University wants me on J1.I am reluctant in getting myself a J1 for voluntary research position because
1. I will always be on J1 after this and
2. will put myself at risk for 2 year home country rule at every step of the way till I get a Categorical General Surgery spot(if at all I am able to get it).

My questions are:-

1. Suppose I match into prelimnary General Surgery Residency this year on H1b.Next year suppose I am not able to find myself a prelimnary/categorical PGY 2 spot,what are my options?Can I enter into research somewhere and will that count towards a total of 6 yrs for which H1b is sponsored?I know that is not possible on J1 as one cannot switch from ECFMG clinical J1 to Research J1 and that person will be subjected to 2 year home country rule.This is the major advantage of H1b,I am presuming and also that one will have the option of jumping to J1(if he gets a position later on J1), if on H1b.Please correct me if I am wrong.

2. If the time spent on research on H1b is not counted towards 6 years cap then its ok but if it is counted(say somebody does two years of research on H1b after Prelimnary General Surgery PGY1 on H1b) then how the entire residency is managed?

Any help would greatly be appreciated.
