Listening Switching Visa for USCE

I'm in the US now with tourist visa for a while and I would like to start building up some LoR here. I'm taking my step 2 ck in two months.

What can I do in order to go to different hospital applying for externship or observership? Do I need to wait until step 2 CS in order to use a specific visa for that?

You don't need to switch your B1/B2 Visa. You can join observerships and externships with it.
