Embarrassed Please help with some definitions! Externships / electives...?

Hi everyone!
Im a medical student from Perú Im in an important moment because its time to decide! I was about to take the Kaplan's prep course for STEP1 in Jauary 2015 But after a lot of thinking i decide that what its best for me is having some clinical experience in USA first and at the same time continue with the preparation for step 1 (i have all kaplan's material for that ) which i will take after the rotation ...BUTTT!! i've read something that really confuses me and its what really means for an IMG the terms ELECTIVES, OBSERVSHIPS, CLERKSHIPS , EXTERNAL ROTATIONS.....i tought i knew what they meant but now im really confused, plese help me!!
Another importan thing is which of these am i allowed to take having into account that im at my last year of med school ( Before the internship ) and i havent take the step 1 and i dont have TOEFL yet :/ (i mean im planning to take it really soon but i do have the FCE that it's like the same but from the UK) Pleae help mee!!
