Post Urgent help needed with medical transcript

I want to apply for clinical electives in some hospitals in US. I already passed my USMLE step 1. And will be in my final year in my college from April,15. I'm having problems getting a transcript. In the last 20-30 years, no one tried applying for clinical electives as a student from our school. As I'm still a student, the responsibility of making a transcript falls on my school, not my university. Since our school never made a transcript, they needed a draft. I was wondering what should I include in my transcript, especially which subject (other then surgery, medicine, gyne)? How should i present my attendences in each ward? Do i need to include marks obtained at ward finals? Please Please Please help my out here

Actually the medical college decides what is to be included in the transcript as there is no particular protocol, at least not in India. My college gives a transcript that gives all the subjects, marks attained in each subject and the number of attempts for successfully completing the subject. That I guess is the important part of a transcript : your grades. However mine has no mention of the ward postings I've attended, they give a separate clinical clerkship certificate for that. So you might have to find out before submitting the documents for the elective you are interested in applying to and ask what exactly they are looking for in a transcript and ask your college to prepare one in that format.
If you need for information search the AMA website on guidelines for preparing transcript and request your college to prepare one in that format.
