Question Which elective to go for?

I have 237/84 in step 1 and doing internship in india.. applying for clerkship now.. need suggestion on which ones to apply..

shortlisted northwestern, mayo clinic, Jefferson Philadelphia, Cornell etc.. kinda confused on what to apply for and which ones i have a chance of getting into.. i don wanna waste money.. already wasted much when i applied n got rejection before step 1.. also have toefl..

need advise..
