Help Externship in Private Clinic

hi all,
i am FMG, already did residency in OBGYN in India, done with step 1, CK waiting for CS result.i am going to an indian OBGYN private doctor in NJ , he is very senior and ready to give LOR to me, i am not sure what LOR should i get from him, observership or externship..?? as i am not yet ECFMG certified am i eligible for externship as he allows me to take history, be with him in procedures.does NJ has any rule specific for externship..??
also if anyone can give me format of LOR for externship, all blessings to him.
one more thing, i know an american private internist, ready to give me observership, is it worth to do observership in IM as i m looking for OBGYN or family medicine. i dont have any options for university hospitals....tried hard but no luck.
thanks in advance, really need an answer .

Hi Dr.,

What you are doing to obtain US clinical experience is quite popular and for may medical students and graduates its the only way. There are major difference between observerships and hands-on clinical rotations.

Hands-on clinical rotations should be completed with the appropriate certifications, liability insurance and under the supervision of an US Attending Physician. Each state has different laws regulating the type of hands-on clinical skills that can be practiced by medical students and graduates under the supervision of an Attending Physician.

If you complete hands-on clinical rotations through a placement agency or on your own they are typically called clerkships. Clerkships are recorded as volunteer work experience on ERAS common application. They are recognized as hands-on US clinical experience and LORs from US Attending Physicians are accepted by residency program.

If you complete hands-on clinical rotations through an educational institution that result in credit/grade, leads to a diploma or job placement they are typically called externships. Externships are recorded as externship work experience on ERAS common application. They are recognized as hands-on US clinical experience and LORs/Clinical Evaluations from US Attending Physicians are accepted by residency programs. This hands-on experience can also be used to apply for salaried positions in the US Healthcare Field that are residency relevant and can be used to apply for residency.

i already finished step 1 , nearly finished step 2 , cs not yet , has a green card ....could u please find me an externship with salary ..iam an IMG and need it urgently also to earn money to pay for the next match ,,i will be in USA through the next 2 monthes ,thank u
