Italy Step 1 and rotations: is it really necessary?

Hi guys. I am an Italian medical student in my 5th year in a 6 year degree. I have already had an USCE (a 2-months-observership in a university hospital in Texas) which was really rewarding and made me think of pursuing a medical career in the US.
Now I am planning and I am in doubt about the best moment to take my step 1. I am really motivated to do a "hands on" clinical rotation before graduating in Sept 2014. I have got a excellent LOR written by the professor whom I spent my observership with. My question is: is it really impossible to find a rotation in a good university hospital with LOR but without step 1? I could take step 1 in the next few months, but I am not completely sure that my score would be high as I want.

It's not impossible but your chances are much higher if you have step 1 score in hand. This is if you are talking about (before graduation) elective rotation.

Are you asking about (after graduation) observership and/or externship. If that's the case, then step 1 score is not that important to find places.

I know it is harder, but it would be even harder to find a residency spot with a poor step 1 score.
I know that LORs are very important. Do you think that would help me to find a rotation too?

Not all hands on clerkships require usmle step1..
