Question Harvard Criminal Offender Record Information

Harvard medical school have something called CORI (Criminal Offender Record Information) form.. which has to be signed by a CORI authorised employee.. i dont know if that means the local police of my area.. or that i have to just fill the form and the authorized employees in Massachusetts will sign it. kindly help..

Default US record not overseas

This is just a form that you fill and give it to them. They'll check your name and see if you have criminal records in USA or not.

It has nothing to do with your home country or current location criminal or Police record.

@appu- can you please tell me wat did you fill in the last 2 spaces of cori form"requested by__________ and photographic identification_________".
and the first para where it says "as an applicant/employee for _____________"

i filled it as an applicant or employee for electives in HMS. the other two, i left blank.. hope that helps
