Default How easy/difficult or possible is it to get an externship?

Without using the paid services.
How about the west coast?

Externships are not easily available without paid services. You really won't find any academic centers advertising this. You can find observerships though.

Also, externships are illegal in California (link here; see page 5). Depending on what you want to do, I am aware of a surgical sub-internship that is available for IMGs at the University of Washington (here's a link).

Im looking for radiology externship or observership or anything that can help me get residency in radiology
