Listening Observership with 1 doctor in 3 places

Hello ,
I m working with an US doctor . He is linked with 1 university hospital and 1 uni aff hospital. Last week he took me in University hospital as well as in his clinic in afternoon . This week he is taking me in uni aff hospital and also in his clinic . He told me , if i work hard he will give me a LoR at the end of month. Is it ok if i put in my CV that i worked or observed in those places in the month of may .... Do i need to put exact date in each places , or how many hours i did .....

You can put it as one entry in your CV, shadowing Dr. Smith in the following institutions: A), B) C)
Mention the number of hours per day and the number of days per week.

One important thing here,
It would be great if your doctor writes the LoR on a LetterHead from the University Hospital, if he can do that then you can title the experience as "observership in that University Hospital", this will have much higher impact than if the LoR was written on his private clinic paper.

Hello step taker ,
He is not going to give me the LoR . As he is the dept head where i am doing rotation with him , he told me he will send it directly to ERAS in July. I am working with him very honestly and doing my best. I think he likes me . But I do not know what he will write. Now I do not have any option. He is a big doctor , i dont think i can tell him anything about it . I know it is risky to waive ... Any alternate idea , how to tell him nicely to give it to me .....

You cannot tell him that. Since he requested that he sends it by himself then that's it.
The only way to avoid that risk is to actually not reminding him about the LoR, comes July.

I am in real trouble ... Doesn't matter which way I go , I told him I will mention externship in CV , then he said ok .... and I did externship in his IM department (4 wks) .... Nowadays getting externship is real tough in a hospital, but he arranged that for me ... If I don't take his LoR, I cant put externship in CV, but If I want to write externship in CV, I have to waive LoR .... I am in double edge sword position ....... risk in either side ....
