Let's give it a quick shot;

By definition these are remote cancer effects not caused by metastasis

Lung small cell --- ACTH --- Cushing
Lung small cell --- ADH ---- SIADH
Lung squamous --- PTH related peptide --- Hypercalcemia
Renal cell ----- PTH ---- Hypercalcemia
Thymoma and Lung small cell ---- AB against presynaptic NMJ Ca receptors --- Myasthenia (Lambert-Eaton syndrome)
Intracranial Neoplasms ---- ADH ---- SIADH
Leukemia/Lymphoma ---- Gout
Pancreas ----- Migratory thrombophlebitis (Trousseau syndrome)
Carcinoid tumor --- 5HIAA --- Carcinoid syndrome
Any lung cancer --- Hypertrophic pulmonary osteoarthopathy
Stomach and other visceral malignancies --- Acanthosis nigricans

It's not 5-HIAA that is secreted by carcinoid tumors and causes carcinoid syndrome. Carcinoids secrete 5-HT (along with other products such as histamine, dopamine, substance P, neurotensin, prostaglandins, kallikrein, and tachykinins). 5-HIAA is a urinary metabolite of 5-HT (useful for diagnosis).
