Default spirited for the beginning

hey everybody i hope every one is doing great ! am just starting my journey of step 1 prep ! its not a piece of cake i know so that is why i wana share everyday of my prep here on this forum! so as to keep myself n others motivated and on the tract! i have started with pathology (done some of the chapters), the reason of which is simply is that i dint wana start with a boring subject , anyone in my boat can join me ! others please keep me motivated and i shall be highly thankful. i ll be posting at the end of the day (oky-not essentially :P )and would be sharing or asking in anyway that suits the situation.i hope i would be supported by all the kind people on this forum

Thumbs Up the beggining is the hardest

hey buttercup786

i also just started my journey in usmle step1 prep , n yes we need to b mighty spirited for it . Ive read few chapters of anatomy and pathology sometime back but ill b going through them again. what study material are you going in for and how are you pacing it all ?

hi, great , we are in same boat! i hope we keep motivatin each other and hope others join us too! as i mentioned i have started with pathology goljan, i have done some of the chapters a while ago and would try to revise them with a new chapter i plan to do each day, i wana finish the chapters listed in pathoma by 21st and i really wana discuss here on the forum about whatever i study each day

#day 1# breast pathology , so read this small chapter today, it wasnt bad ,i hope it doesnt seem odd and inappropriate if i share what i learnt today , inflamatory conditions gave some msgs like acute mastitis almost always in a lactating woman, due to all those cracks n fissures and yes think of staph wen it comes to the bug of acute mastitis and a type 1 diabetic having multiple hard nodules lymphocytic mastopathy should come in mind, and yea smoking screws up the breast too causing the squamous metaplasia of the lining cells of ductules n lobuels (thats cuboidal normally) so a smoker lady presneting with a painful subareolar mass --blame the cigarettes! and yea the duct ectasia the loose dilated duct in the wrinkly loose old ladies :P oky not that old just to memorize cz it mostly occurs in postmenupausal women

and then the famous of all the fibrocystic changes! they are way comon! and if its the blue domed cysts and fibrosis , u r allll set -- nothin to worry (no cancer risk) well if thats sclerosing adenosis u got a lil risk ,and oh man if its atypical hyperplasia 5x risk .... its nice to think of the fibrocystic changes like proliferative (adnosis, ductal hyoerplasia, atypical hyperplasia) and non proliferative(cycts, fibrosis)...


and then those benign tumors -- only 3 of them mentioned fibroadneoma( who doesnt know u ) mc tumor being diagnosed in yng ladies, free as a bird --oky to be precise freely movable like a marble and it is innocent no cancer risk, phyllodes tumor is elder sis of fibroadenoma :P -- means tho from the same family( arising from the stroma ) it occurs in elderly women , and ooops angry elder sis (it can transform into cancer) .... then the intraductal papiloma, a fibrovascular like outgrowth lined by both epithellia ! now this is imp clue distinguishin it from cancer! cancer wont have both epithelia! ! the myoepithelial layer wont be there in cancer ..


then comes the breast cancer! mc cancer in women by incidence and scnd most comon cancer cause of death in women , so DCIS n LCIS havent gone too evil yet -- means they havent crossed b.membrane - oky so dcis shows up with microcalcifications ( microcalcifications are also seen with traumatic fat necoris , sclerosin adenosis) so how r u gona know its cancerous or not-->may be go for a biopsy particulary if the calcifications are tightly clustered/punctate/branching.......then the mc and worst -- the invasive ductal carcinoma--- the griity hard (stone hearted bad cancer :x ) but some of its kids are gud :P like tubular ca (the one in terminal tubules with those teary eyes :P the tear drop like ductules) and mucinous (the one in soft old ladies (just to memorize cz it occurs in oldies and is soft n the malignant cells swimming in mucinous pool) and medullary cancer ( the one with sheets of cancer cells among the lymphos) thats not that bad too!


and for that scary looking inflamatory breast cancer-- one word for it-- it screws up the dermal lymphatics so thats the reason for the skin changes the orange peel like appearnce n all that. thats really bad if somebodys having this ,worst prognosis and caution! dont send the patient home with antibiotics cz it looks like mastitis sometimes, but isnt warm to touch unlike mastitis i guess. yea and one point i read somehwere else -- is that Inflamatory breast cancer is positive for rhoC


and for gyncomastia causing drugs DISCO was a nice nmenonic digitalis, INH, spironolactone, cimetidine , oestrogen tho u know there r many other drugs that can cause this benign glandular proliferation of breast tissue


so thats it for today , i hope my posts dont waste anybody's time , guys if i dont do this i wouldnt be able to stay focused. i had started a couple of times and screwed up -- dont wana get off track this time.... so i just hope my posts dont annoy anyone. . hoping to be more productive tomorrow

hey buttercup786, well your post is commendable (y) ... summarised thr whole thing in brief . read d whole thing . thanxxx

u mentioned uve started a cpplo b4 but it got screwed up...sadly the same things happening to me .....ny suggestions in dat regard ? also i am still very unsure on how much to cover ideally in 1 day...wat wud b the ideal target..umm say in pages per day???
