I agree 5 y/o girl with multiple bruises on the abdomen and thorax

A 5 y/o girl is brought to the ER by her parents because of abdominal pail. Physical examination shows multiple bruises on the abdomen and thorax in different stages of healing. A chest x ray shows hiarline fractures of 2 ribs. The parents say that they do nor know how she developed the bruises or the fractures. Qhich is the following is the most appropiate next step?

A) Call the police immediately
B) Hospilize the child for further studies
C) Notify Child protective services
D) Obtain a promise from the parents that they will not strike the child again
E) Separate the child from the parents

Correct Answer correct answer

The correct answer is E) Separate the child from the parents

While there is still some possibility that the child has a medical condition (osteogenesis imperfect), the signs suggest child abuse. Based on this suspicion the physician must act.
First protect, then report!. After the child is separated from the parents, then report the case to child protective services.
Remember, the physician is not judge and jury here, but a screening mechanism.

Kaplan.
