Default pharyngeal pouches

A newborn is found to have facial abnormalities as well as absent thymus and absent parathyroid glands. A cardiology consult suggests that the patient had abnormal formation of the aorticopulmonary septum. Which of the following pharyngeal pouches failed to differentiate?
A. Pharyngeal pouch 1 and 3
B. Pharyngeal pouch 1 and 2
C. Pharyngeal pouch 3 and 4
D. Pharyngeal pouch 2 and 4

Correct Answer

The correct answer is C.
The patient in the question stem has DiGeorge Syndrome, which occurs when pharyngeal pouches 3 and 4 fail to differentiate. Pharyngeal pouch 1 differentiates into epithelial lining of the auditory tube, middle ear cavity, and mastoid air cells. Pharyngeal pouch 2 differentiates into the epithelial lining of the palatine tonsil crypts. Pharyngeal pouch 3 differentiates into the inferior parathyroid gland and thymus. Pharyngeal pouch 4 differentiates into the superior parathyroid gland and ultimobranchial body (neural crest cells migrate into the ultimobranchial body to form parafollicular cells C-cells of thyroid), which secrete calcitonin.
