A 45-year-old African-American female presents to the emergency room with a change in mental status. She was brought by her sister who states that the patient has been extremely tired and has lost 40 pounds over the past month. The patient is taken to the intensive care unit and stabilized. Blood work is sent to virology, which shows a new type of virus that uses T helper cell 1 as its vehicle, but is destroyed by T helper cell 2. Amplification of which of the following interleukins would best help this patient recover from the viral infection?
A. Interleukin 4
B. Tumor necrosis factor alpha
C. Interferon-gamma
D. Interleukin 5
E. Interleukin 12

IL 4 stimulates the TH2 cell proliferation n inhibits TH1 cells

Correct Answer answer

The correct answer is A.
The patient has a virus which uses T helper cell 1 as a vehicle and is killed by T helper cell 2; therefore, amplification of interleukin 4 would best help her recover since it promotes synthesis of T helper cell 2 and inhibits synthesis of T helper cell 1. Interleukin 12 promotes synthesis of T helper cell 1, which would worsen her condition by providing more carriers for the virus. T helper cells produce interferon gamma (which inhibits T helper cell 2, but has no effect on T helper cell 1) and tumor necrosis alpha. T helper cell 2 produces Interleukin 5, which is not better than Interleukin 4 for eradication of this viral infection.
