Syringe Complications of IV Drug Abuse!

A 31-year-old female presents to the emergency room with an abscess in her arm. She admits to shooting heroin into her arms. Prior to having the medical student debride her forearm, she is counseled that continuing heroin use puts her at risk for all of the following except:
A. Right sided endocarditis
B. Seizures
C. Adult respiratory distress syndrome
D. Pulmonary edema
E. Hemorrhoids

hemorrhoids?
not sure, the presence of 2 respiratory symptoms makes me strike both out, left with seizures which i think heroin will cause, endocarditis is def true.

e may be due to constipation I guess, so vote for b (heroin doesn't lower seizure threshold, unlike antipsicotics, antidepressants and CNS stimulants)

Default B. Seizures

heroin is an opioid and that depresses everything (coma and respiratory depression), so i dont think u can get seizures. i rmr something about ards and hemorrhoids and its an iv drug. I dont know about pul edema but i think B is a better answer.

morphine is drug that being used for pulmonary edema maybe heroin too!

Heroine doesnt cause seizures, they cause sedation.

You guys, it causes SEIZURES, thats why you give IV diazepam for cocaine-induced seizures ( Lippincott's Pharmacology).

I accept , A,B,D.

I am not sure about E and C.

From what I read, I think its C.... right ricko??? - its not inhaled....

Answer should be Pulmonary Edema

- it cause seizure, because it decrease the threshold of it.
- it cause R Endocarditis, because of the obvious .
- It cause ARDS, because it can irritate the pulmonary bronchus and also patient with seizure can develop aspiration.
- It can cause Hemorrhoid, Constipation is one of the main effect of opoid, can lead to hemorrhoids.

Look over FA 2011 page 450 and 451. It will help you rule out some of the option given in this question.

Also check Pharm on Kaplan books page 150. Even on Kaplan books it will tell you the answer of this question.

The correct answer is B.
Users of heroin are at risk for hepatitis B and C, HIV/AIDS, abscesses, hemorrhoids, right-sided endocarditis, adult respiratory distress syndrome and death from respiratory or cardiac arrest or from pulmonary edema. Seizures are usually not associated with heroin use.

Nice question, but on first aid it clearly says seizure listed as one of the side effect. And on kaplan it says can be use for pulmonary edema is safe.

it should b B or D ... i will go with D pulmonay eedema i dont think ... as it acts at opiod receptor so it will narrows airways and can cause copd like complications but i dont think there will b pulmonay edema ...

I go for "Seizures"

Many drugs ranging from illegal drugs such as heroin and cocaine to aspirin and chemotherapy drugs are known to cause noncardiac pulmonary edema
