Nerst equation is for calculating how much potential you need to stop the diffusion of any given ion through a membrane.
Example: you got high sodium (Na, a positive ion) in the extra-cellular fluid and low sodium inside of the cell, a gradient forms because of the concentration difference and Na goes inside the cell but there is a moment where the inside of the cell is so positive that the charge repels the Na ions even if there is a still a concentration difference
