Thumbs Up Indian med student aspiring for USMLE

I'm in the 3rd year of my 5 and halfyears study of MBBS in India. I am passionate about doing my residency in US. I just want to know the following :
1) when will be the right time for me to take the STEP1 ?
Though I am fresh from the basic sciences ( that are required for the STEP1) I'm not sure if I'm really ready or capable enough to take the Step1 before my internship. I heard that preparing for usmle differs a lot from what I study during my MBBS.
2) Does taking a TOEFL give us any advantage ?
3) surfing the sites i came to know that 'ELECTIVES' have some advantage in Usmle. What are the prerequisites for an Indian medical student to do an elective in US ?

( My areas of interest are M.S.ortho , Radiology and general surgery )

I'd be really thankful...Please help an inspiring aspirant.
