Default repeated usmle question 23

You receive a phone call from the nurse of a nursing home telling you that your patient refused to take her medications for the third day. She is 75 years old and had a stroke 5 years ago resulted in disability of both lower limbs.
What is your best response to the nurseís concern?
A- Ask the patient for the reason of refusal and donít force her to take her medications
B- Get help of the staff in the nursing home to force the patient to take her medications as you are responsible for any complications that could happen to her if she doesnít take her medications.
C- Call her family and address this issue with them to find the best way to solve this problem to avoid any blame if something happens to her
D- Tell the patient that if she doesnít take her medications then, there is no need to take any future appointments

but the funny part is the stroke that lead to disability in both LL!!!

Let me ask you a question that I really like

A previously healthy 35 year alcoholic man present to the ER with hematemesis and frank blood after retching for a couple of hours, the patient has chest pain. Vital signs stable and there's no organomegaly nor weight of loss. What is the cause of the bleed?

A-rupture of a vein
B-rupture of an artery
C-esophageal ulcer
D-esophageal cancer
