Neuro Brain biopsy showed a few lacunas in the centrum semiovale, left basal ganglia, and left internal capsule

A 60-year-old female with long-standing hypertension was referred for a 2-year history of progressive gait difficulty, generalized slowness, hypophonia, and infrequent urinary urgency and incontinence. The ability to do everyday tasks such as managing finances, preparing a meal and driving has also become very difficult for her. Her illness began with a stroke-like episode 2 years ago, with acute onset of a mild right-sided hemiparesis, starting first in the arm and later involving the leg; there was mild dysarthria as well. Before this hospitalization, she had intermittently experienced several episodes of blurred vision in the right eye, each lasting 1-2 min. She had been a one pack/day smoker since the age of 23. CT scan showed a small lacuna in the head of the caudate on the left, and patchy periventricular white matter lucencies. Brain biopsy showed a few lacunas in the centrum semiovale, left basal ganglia, and left internal capsule. There were no Lewy bodies found there, in the locus ceruleus, or in the nucleus basalis of Meynert. What is the most likely diagnosis?

A) Alzheimer's disease
B) Sporadic Parkinson's disease
C) Binswanger disease
D) Pick disease
E) Huntington disease
F) Von Economo encephalitis
G) Multi-infarct large vessel vascular dementia

How can we differentiate between, Binswanger disease & Multi-infarct large vessel vascular dementia??
