Arrow Step 1 Score (230/83) + How I Did It

hi guys,

First of all, thanks to everyone for their support and to those individuals who posted their stories before me and showed all of us the right way to study and prepare for this test.

So, I took the test on October 12th and got my results on November 2nd, exactly the third Wednesday after the test, which was also a Wednesday.

The result score sheet was two pages, the first one indicating three things: (1) pass/fail; (2) 3 digit score (3) % questions correct

so here is the breakdown:

(1) Pass
(2) 230
(3) 83%

What and how I did it?

Time spent total - exactly 2 months
materials used - First aid 2011, usmle world qbank, NBME exams, doctors in training program

Why did I do it that way?

(1) first aid 2011 - i found that 95% of the material tested on the exam comes from first aid; hence it is must have tool that must be known by heart, with all the facts and figures. there were some specific questions on the step that i remembered learning from the book itself. if all the details are known, then there is a limited chance that one will perform badly
(2) usmle question bank - i chose this bank over kaplan, not only because it is the most recommended one, but also because it is better, more focused and prepares you well. the questions presented in the bank are challenging at times and makes you think, adjusts your brain so that you can think differently, which is exactly what the step aims for. with respect to kaplan, they test on random details that make you feel glad that you learned them, as they are often missed. however, they are useless most of the time. i also did these questions each morning 46, random, timed and unused to maximize my test taking abilities
(3) nbme tests - i did nbme 5,7,11,12 - i have written down stats below. i think these tests are harder than the actual exam, but certain ones, like the newer ones, are more apt and correct and predicting the actual score. plus, its good practice for knowing your weak spot
(4) DIT (doctors in training) - i haven't heard many people write about this here, but i did it. i got the downloaded copy and i printed out the ~300 page pdf document and got it bound. i followed the program religiously and it paid off. i saw my scores go up in nbme testing and it reflected on the actual test as well. i am sure that had i done it another time, my score would have gone up a bit higher as well. it is the one of the best sources out there, as brian jenkins is good, focused and attuned to what is most commonly tested and makes you well prepared for the test. i did this program, which lasts 15 days 20 days out from the test.

Relevant statistics:

usmle world - cumulative avg. 68%
usmle world 1 - 247
usmle world 2 - 245
nbme 5 - 208
nbme 7 - 219
nbme 11 - 227
nbme 12 - 233

(name taken in the following order, first to last: 5, 11, 12, 7)

bottom line: nbme 11 and 12 have higher correlation

important point - make the investment of doing these nbme tests online, especially either 11 or 12, if doing only one. usmle world simulation tests overestimate by a large extent, so they are unreliable for prediction, but good questions to practice. in comparison, NBME tests are better and worthwhile the time to spend.

p.s. one LAST thing i want to mention - PLEASE PLEASE PLEASE don't spread yourself thin. there are many people who will go to various books to study for the test. the true one source out there is first aid and usmle world. don't go crazy with Goljan or some other resource. use that for better understanding a concept rather than memorize particular details on a subject or organ system.

a) How were your basics when u started out and what kind of student in med school?

b) where did u get DIT videos and notes from?

c) how many hours did u study/day?

Hey congratukations!! Awesum score. M sticking to fa and uw too. I did dit one day. Found him really boring. Wat makes u stress on dit somuch?! Is it some extra xplanations. Or that the dude makes u memorize fa even more?! I still applaud u on getting that score in 2months!! R u an amg or img?!

a. i am a caribbean med student ... got done with 5th semester in august and started studying right there. but from april-august (which is 5th semester), it was an intro to clinicals, hence there was no studying of step material

b. i got the DIT and pdf from my friend, but you can download it via searching google for DIT videos 2010 etc.

c. i studied about 7-8 hours a day and took 1 day off every time i took a practice length test.

i followed the DIT program and did all the information/questions given in the handout. it is a 15 day program, with 3 videos dedicated to each day, which total to about 4 hrs; but with pausing and doing the quizzes, takes 6 hrs i would say.

so what i did is that i had done FA on my own for the first month alongside UW and then i saw that my score had not gone up, so i changed strategies and did DIT, where i followed it with the FA book, as the program covers it front to back. and i did the quizzes as instructed by the DIT program.

they have stopped giving the 2 digit score.

they tell you whether you pass/fail; they provide a 3 digit score; they tell you the % of questions you got correct on the test.

they say that 75% correct = 188 ... but that makes no sense as i got 83% with 230, so they must have some fuzzy math.

they also give the breakdown on the subjects/organ systems like the nbme

hey thanks ... i understand and agree to some extent that DIT is boring, but its worth it and there is a reason why its very popular amongst US medical students.

i stress DIT because when we study on our own ... we tend to overlook some small things or forgets some details. hence, DIT brings it in all together and makes it all that better.

I've also completed DIT and although it took me over a month to do it I think it has really helped. I'm currently doing uworld and reviewing FA again. My test is at the end of December. Would you suggest that I do DIT a second time?

Its not its not and its not 83 percent its just the two digit score

The Usmle already annouced that from now on the two digits score related
to a specific 3 digit score will be substantially lower

and this annoucement can be seen on the usmle . org

I THINK THE GUY IS TELLING THE TRUTH ABOUT HIS SCORE.............AND ABOUT YOUR CONCERN THAT TWO DIGIT SCORE WILL BE SUBSTANTIALLY LOWER........LET'S LOOK AT THAT ARGUMENT.......

IN THE PAST THAT IS BEFORE OCTOBER 1...230 WOULD HAVE BEEN EQUAL TO A 97 TO 99.......

AND NOW THE GUY HAS GOT 83.....SO HIS SCORE IS ALREADY LOWER.....ALOT LOWER.........AND SINCE PASSING IS STILL AT 76%...IT MAKES SENSE..........

Help two or three digits??

This means that now the concept of two digit scores will completely go away cos the author of this post mentions he not getting one in his report.

I hope the Programs will over time realize this and make their cut offs according to the three digit scores.

Otherwise a person getting in 80's as his two digit score or whatever its called will be below a guy getting a 90 who took the exam before Oct 2011 in spite of having a higher three digit score.

Lets hope that does not happen.

Ok i also got my score report ck score report according to the new 2 digit 3 digit corelation with 285 99 and there is no percentile or % written on it

Default state medical boards

the two digit score has always been the requirement of state medical boards

and only for them it has been created other wise its the three digit score that is the real score of a person in time and space

and no one can dare to mess up with the state medical boards

anf their requirement is that to apply for licence( after step 3} you must have atleast 75 ( not 75%) on your step 1 , step 2ck and step 3 as a
2 digit score ( they never cared about 3 digit score) and a pass on step 2 cs so who can dare to report 2 digit scor in % . IT CAN NEVER BE IN PERCENTAGES OR PERCENTILES IT MUST BE JUST A 2 DIGIT SCORE( AND NOT A % , PERCENTAGE OR PERCENTILE).........AND THATS WHY JUST BY SEEING 83% MY MIND COULDNT ACCEPT IT AS A TWO DIGIT SCORE

So Samikhan can you tell me how is it possible that a score of 230 is equivalent to a two digit score of 83?

Do you mean to say that ppl who took the exam before Oct 2011 will have a higher two digit score ,no matter how low their three digit score is actually?

Then that would mean that if programs screen applicants only on the basis of two digit score applicants who took the exam after OCT 2011 will have a tough time getting an interview call.

So to conclude i think what the author of this post has written about not getting a two digit score seems true.

Any others who have recently got their score????

The confusion arises from the fact that the original poster wrote "83%", where it isn't really 83% of the questions correct - he probably meant 83 on the two-digit scale, like it usually shows up in the score report (As usual, no one really knows for sure how many questions correct that would be, but there are estimates).

Two-digit scores have been changed effective October 1, 2011. See below. Also, see the official announcement on usmle.org

Yes, for the most part. Anything >240 used to be 99. Now, a 265 is like a 90 or something. The two digit score has always been, and continues to be, for the most part useless. It is only there, like others have posted above, for state medical boards.

That is why two digit scores will no longer be reported to ERAS/NRMP. Programs will only see applicants' three-digit scores. It's much more objective.

Hi, I read ur story, thank u so much . I need ur help unfortunately this is my 2 nd time for step 1 and I did fail and and my score still low in the 60th ( 61, 64) on review my score report I found I am weak in General principle of health and diseases , from where I can study that issue. I did before USMLE Q bank 2 times and 70 to 80 % FA and some Kaplan lecture , I getting tired from this exam . Do u have any suggestion . Please do if u can . unfortunately I have limited time to study because I am working full time.

IF IMGs like the author of this post dont know the meaning of 2 digit score who else are gonna know it.....

IMGs are the ones dying for a 99( whether they get it or they fake it) while AMGs only care about the 3 digit score

So how can we say the author probably didnt know the meaning of 2 digit score.......IMGs must know the meaning of 2 digit score and these are the ones who are ****ed up now as how are they gonna explain to their family about not getting a 99 ...............actually its all the blame of the society which understands 99 as 99% whowww our son is the father of einstein
as he corrected 99 percent of question in a tough American exam...Thats amazing???????and the second party to be blamed is the organization which is so unpredictable about scoring

okk.............. so first of all, no need to make this personal.

second, i take no pleasure in lying ... i write the truth and that's it and i get no joy in writing something that will not help someone.

i wrote my original post based on everything that i read from the two page score sheet that i got.

it gave me a 3 digit score - 230
it gave me a 2 digit number --- 83 ... which was not the "percentile", rather the % of questions i got correct on the test ... this is what is written.

i can read, write and speak english ... so i would know the difference!

so regardless of the theories of the 2 digit score ... i know i got a 230. now, as most of the people here know ... a score of such one year ago would be written as 230/97 or 98 ..... now, they have done away with the 2 digit score.

they tell you the % of questions you get right.

I apologize to both samikhan3232 and shriman, the original poster. I did not mean to say that shriman didn't know the meaning of the two digit score, but that he may have written a % sign by accident. I haven't seen the new score reports yet, so I don't know what it says next to the two digit score, but if it does say that it represents percentage of questions correct (and not that it simply _correlates_ with percentage of questions correct), then sorry! Even the official announcements only say that the "scale was changed", not that it reflects something different altogether. 230 is great in any case. I am of the opinion that two digit scores only add to the confusion (I don't tell my family my two digit scores, what I do is draw a bell curve and tell them where I fall, and that is good enough for them)

If it does represent the exact percentage of questions correct, then we are technically closer to understanding the scoring scale. However, if this is true, the number of questions correct and incorrect between a 230 and a 260 is not too different, which is why I kept thinking that it can't be a percentage of correct questions (I've seen 243/85, for example) But again, I haven't seen the newer score reports. Congrats on your score and keep it up


addendum/edit:
just saw a score report from October 5, and next to the two digit it says "This score is also determined by your overall performance on the examination. A score of 75 on this scale, which is equivalent to a score of 188 on the scale described above, is set by USMLE to pass Step 1." This is the same text I had seen on other score reports. No sign of percent anywhere, but anyway the reports may have been changed later, who knows. I don't know and I don't mean to argue over simple stuff like this, just pointing out that this is the first place I ever see that.

Default help about the score

Hi shriman!

I too got my results today, and I am a bit confused, so please help me a bit if you can. I got "pass" then 255 and then 88. Now I know 255 is great, but i have seen many get 99 (two digit) with just 230 score. Then why only 88 for me (or 83 for you)? Are you sure this is the percentage of correct answer?? Is there any new system of scoring? Please let me know about this.

hey congrats on the great score. in the "old" system, where the 2 digit score was looked at, your 255 would be equal to a 99, meaning your score is in the 99th percentile and you did better than 99/100 students for example.

now, i think, based on what was written on the report, that the 2 digit score is gone away with and they have instead decided to write down the % of questions that one got correct. so in your case you got 88% write.

now, i am unsure whether there is any calculation that tells what 3 digit score one would get based on % of questions answered correct ... but your score is awesome and the rest is nonsense!

i did DIT myself and got same 230/83...although i took lot more time to prepare but i will say DIT is the real thing for step 1!

This is one of the best prep-articles i've ever seen on this forum! Thank you so much!!

I truly believe so as well, FA and UW are a very powerful combination! If used well, they can help us get that amazing score.

Just wanted to ask you, regarding DIT, i have a month to my exam -during which i plan to do FA twice- and im thinking of doing DIT with my final/last FA read.
Is it manageable to do them in 15 days? How did you do it, i mean how was ur daily schedule during ur "DIT phase"? Did you watch and then read, or just watch?! any questions?! etc... :S

I'm afraid if i do them, i'll take too long and i wont manage to go through FA twice :S

Would love to hear ur thoughts on this since our strategies are so alike!

Congrats on ur amazing score, hopefully i'll be in ur shoes soon!

i think its the best time to do DIT 1 month out before the exam. make sure you take nbme tests to see where you stand before you start the program, halfway and after so you know you made the most of it.

15 days covering FA is pretty good, with many other tidbits, which forces you to know all.

what i did: i printed out and got the pdf files bound and then watched the program day to day, so day 1, then next day its day 2, etc. i would follow the videos, do exactly what Jenkins (the preceptor) told. after i was done with the program for the day (and sometimes it could be long) ... i would review the material and go to bed; each morning as always, i did 46q from usmle world and go over them (so that remains unchanged); at the end of the week, i would review all the material from that week. also, i would take the nbme after day 8 to see where i stand and how much i improved.

I feel good about DIT so i think im going to make my plan revolve around it.
