Question i have one name....

**The information you provide to obtain a USMLE Identification Number will become a part of your permanent ECFMG record.**

Biographical Information

Name

If your correct and current legal name consists of one name only, you must enter this name in the “Last Name” field. Do not enter any characters in the “Rest of Name” field. Your request for a USMLE/ECFMG Identification Number will be issued only after ECFMG verifies that you only bear one official name. You will receive instructions on how to verify your one name only by e-mail.

If your correct and current legal surname (family name) consists of more than one name, enter each of the names in the Last Name field.

ECFMG ON-LINE AUTHENTICATION PROCESS
Refer to the Instructions below.

If you do not answer accurately, this may result in a finding of irregular behavior. See Irregular Behavior.

INSTRUCTIONS

If you have previously submitted an application to ECFMG, you must answer "Yes" to this question. You must answer "Yes" even if you submitted an application under a different name or you did not take the exam for which you applied. You must answer "Yes" regardless of whether you submitted an Interactive Web Application (IWA) or a paper application.
If you have never submitted an application to ECFMG for any examination, answer "No".

i have submited an aplication to ECFMG but what i have submited thats not correct bc i have single name and i did mistake i entered mine that one name in first name,now what i do bc in instruction they are saying that you must answer "yes" if i submited for an aplication,and i have also got USMLE Identification Number? what i do, i submit the another aplication with answer yes?

Do not submit a new application. Instead request a name change. You can download the form for requesting name change from OASIS.

In that form you will have to request a name change giving the reason for the change and attach a valid document in support of the new name like a passport.

ECFMG will update your new name within 15 days.

please tell me i dont have any idea about that how can i request for name change,and i got usmle
identification number on 21st nov 2011 and i havnt proceed that one usmle identification number i think now it may be expired?

Log into your OASIS account using your usmle id and password.

In the section of Applicant Contact Information you will find a pdf to request a name change.
