Listening IMG with NBME Form 12 score of 610 (247) ready or not?

Hi guys,

I'm an IMG from Ireland, my exam is in 2 weeks and i wrote the NBME Form 12 today. I scored 610 which gave me a predicted score of 247. I want get a residency in an East Coast city (nyc/boston/etc) in general surgery.

My prep for usmle has been limited and it would be difficult for me to postpone the exam both due to hectic coursework. Do people think if i replicated that score on the actual exam that it would be enough? Or do i need to reschedule?

P.s. I ask because I have heard rumours that IMGs need to score way higher than AMGs to secure similar residencies?

Peace

Thanks a lot for your reply add1. Is there any point in me doing nbme 7 in your opinion? Would my form 12 correlate well enough with the actual mark do you think?

I ask because I feel like my time would be better spent reviewing high yield material than doing another "score predictor" exam that's not going to teach me that much!

Nice score!

I read somewhere that if you get similar scores on at least two NBMEs you are ready to write the exam. But I'm not sure, someone whose written the exam would know better

Best of Luck

1. While NBMEs are very predictive of your actual score, you gotta correlate them with at least two NBMEs.
That 610 is a very good grade that shows you're well-prepared. However, take another one just to confirm that the 610 wasn't just luck. I got very similar grades on four NBMEs, but close friends of mine had grades that were all over the place. People will say "12 is the most predictive" or "7 is the best", in the end it's all anecdotal. Yes, they are very predictive, but basing yourself on 1 test that only has 200 questions is a bit of a stretch. I think an average of as many NBMEs as you can take will give you the best estimate of your grade. Maybe form 12 coincidentially had many things that you felt were very easy, and so you easily got that 610. Maybe the concepts that were tested were very hard for you, and you still got that 610 (and maybe you deserved more!). See what I mean?

(I got 590, 570, 590 and 590 on forms 6, 7, 11 and 12. Two of them were school-sponsored btw, I wouldn't have spent all that money just on NBME tests. I thought it was uncanny that I got the same score three times. But I'm still waiting for my real test results)

I strongly recommend you take at least another one, but you don't have to do it now. Like you said, it's not like you'll learn much from it. You could invest the time into studying for a few days and THEN taking another NBME.

There are correlation tables and formulas online that people use to predict their scores from question banks (I suppose you're using some form of questions). I don't recommend trusting them 100%, but you can at least tell whether you're failing, passing, passing comfortably or if you're going to do very well on the exam. You can combine the predictions from NBME(s) together with the not-very-precise predictions of question banks.

If you're going to reschedule then do so soon so you save as much money as possible.

Side note: UWorld offers their self-assessments which are cheaper than NBMEs, but I have seen them overpredict one too many times. I didn't use them, personally

2. AMG vs IMG grade:
As IMGs (I'm one as well) we have several things against us in the whole residency application process. First, most of us need a visa = extra paperwork, more expenses and more effort to make an IMG a resident. Second, residency programs know that most US medical schools are at least governed by similar standards of quality (not necessarily, but go with me on this). Not all foreign schools are good (I know mine is terrible). You can't necessarily trust a 4.0 GPA from a foreign medical school. Third, many IMGs take quite a bit of time to study (more than the 1-2 months most AMGs study) so they know IMGs usually have more time to prepare themselves.

In conclusion, yes, sometimes a good grade is needed to set you apart if you're an IMG. Usually you'll need to achieve at least the average of what the typical AMG applicant to that specialty gets. Two important caveats: a much higher than average grade does not guarantee a spot, and a low grade does necessarily preclude you from it (although a very low one definitely will). That's why you have the rest of your application to support you as a candidate (research/publications, any work experience, rotations/evaluations, US clinical experience, letters of recommendation, your dean's letter)

Good luck on your studying - study hard and study well
