A 47 year old woman presents to her physician complaining of sudden pain of her left hand associated with numbness for three hours.

Definitely not D.
Can't be A either because then she would have atleast some history of diabetes/hyperlipemia etc.

Most likely ans is B.
Irregularly irregular rhythm and history of palpitations suggest Atrial Fibrillation in which arterial embolism is common.
