Virus Infant Immunity Against Maternal Viruses!

A 25-year old women is exposed to a virus during the first tri-mestester of pregnancy. The women gives birth to an infant who is deaf and has cataracts, a heart murmur, and microcephaly. If present, which immune function would have protected the infant from viral disease?
a) inhibit viral replication by IFN
b) inhibit viremic spread by antibodies
c) killing of virus infected cells by CD8
d) killing of virus infected cells by NKs
e) uptake and neutralization of the virus by macrophages.

The answer is B.
Inhibition of viremic spread by antibodies. The infant's clincial manifestsations are consistant with diagnosis of congenital rubella. The key is cataracts. Congenital CMV infection has many of the same symptoms but its not associated with cataracts. If the mother had been immune to rubella virus before she became pregnant, she would have generated antibody upon re-exposure to the virus, this antibody would have prevented the viremic transfer of rubella virus to the fetus. IFN may limit local replication of rubella virus but would not prevent viremic spread of the virus to the fetus. CD8 and NKs may cause local killing of cells infected with rubella virus, but they also would not prevent viremic spread of the virus to the fetus.. Macrophages arn't helpful in preventing rubella to the fetus either.
