Hello everyone...

During the next few weeks my goal is to read First Aid a few times and review along with Uworld...

Every two weeks I MUST take one NBME ONLInE to see how it goes...
To be honest I'm very scared....

Default good Morning....

Today Endocrine, GI and Hematology/oncology at First Aid...

🎼🎶📝😱📚📒 Yesterday I took me a long time to finish my reading...
Let me share a question from yesterday Qbank
R. E. A. D. Y ????

A 24 yr old female with no significant past medical history presents to her primary care physician with exercise intolerance. She has been trying to exercise to lose weight but becomes short of breath after only minimal effort. Her temperature is 37.0, blood pressure 110/70, heart rate 110, respiration 20, and oxygen saturation 94% on room air. Physical examination reveals normal jugular venous pressure, normal breath sounds, a II/IV systolic murmur at the left upper sternal border and a fixed split S2 heart sound. Laboratory studies are normal. ECG reveals normal sinus rhythm, a right bundle branch block, and left axis deviation. which of the following is her likely diagnosis???

A-Ostium Primum atrial Septal Defect
B-Ostium Secundum atrial Septal Defect
C-Ventricular Septal Defect
D-Patent Ductus Arteriosis
E-Ebstein's Anomaly

Please feel free to share your answer and discuss the question and answer...
Remember is a LEARNING tool we learn by practice and practice makes YOU perfect.... Good luck

if know one participate That's ok... We ALL need to study so I'll be posting the answer tomorrow morning...

enjoy your day.... Don't give up and let's read... 👀😃🌸😇👀🐼🙈

Hey. Good luck on your exam. I am taking mine in November or December.

Right now I am revising Anatomy/Bio/Micro/Pharma/Immuno. Small chunks of each everyday.

Default Hello hello...

First of all thank you Coliseum for the GOOD wishes, please keep in touch check the questions once in a while it might help you. I'm glad to know someone is taking the exam around the same time as me.

MOM260 you are absolutely right... We Girls can do a lot in less than 24 hours... It seems like we don't get sick like if we have some power or something to keep us going...

Thank you both for your comments..

Today I woke up at 5:30 am had doctors appointment at 7am and turn out to be at 8 am and the second appt at 11am so my morning was not good... NO reading until now..

I must finish reading 12 more pages Hemato/onco
After that Musculoskeletal and uworld 44 questions and review...
I feel the day is not enough too many things to do at once...

Let's not waste more time and begin reading...

Ooohhh the answer of the question...

24 yr old female with no significant past medical history presents to her primary care physician with exercise intolerance. She has been trying to exercise to lose weight but becomes short of breath after only minimal effort. Her temperature is 37.0, blood pressure 110/70, heart rate 110, respiration 20, and oxygen saturation 94% on room air. A-Ostium Primum atrial Septal Defect
B-Ostium Secundum atrial Septal Defect
C-Ventricular Septal Defect
D-Patent Ductus Arteriosis
E-Ebstein's Anomaly

Please feel free to share your answer and discuss the question and answer...
Remember is a LEARNING tool we learn by practice and practice makes YOU perfect.... Good luck

Answer is A.. OSTIUM PRIMUM Atrial Septal Defect
Why... An atrial septal defect is an abnormal communication between the left and right atrium. Is a congenital defect that initially causes blood flow from the left atrium (higher pressure) directly to the right atrium (lower pressure) resulting in a shunt.
Ostium Primum atrial septal defect has a right bundle branch block and LEFT axis deviation on the ECG, AND
Ostium Secundum has a right bundle branch block and RIGHT axis deviation.
At the physical exam the question mentioned a fixed split S2 heart sound... Remember in a patient with atrial septal defect you hear a fixed split S2. Normally the split S2 is during inspiration...

Now let's keep reading,
Enjoy your day..
