Hi, rose!

I'm not sure if I got your question right but here goes... This is premised that the usmle is very much the similar to uworld. In uworld, the stem specifies the virulence factor they are asking from a pathogen. It's either directly or from the case. For example, if a bacteria has many virulence factors, the case would reflect or the question stem would ask about "the way the organism evades the immune system", "how it colonizes a specific tissue", "virulence factor responsible for the high infectivity or pathogenicity", etc. It boils down to being aware of the function/contribution of each virulence factor.

As for the list of virulence factor for each bacteria, I find that Kaplan mentions the important ones, or at least, most of the ones asked in uworld. If you're looking for a black-and-white list, you can find one in CMMRS. There's a column of virulence factors for each bacteria in tables found at the end of every chapter.

I'm not sure if there's an online book. I got mine from a local bookstore. Maybe you can find a pdf online... It's a pretty popular book. You might also want to check if your bookstore has it. I got mine really cheap (US $7).
