Arrow NBME 5 Question help

So there is an elderly pt who comes in with shortness of breath and has smoked 2 packs of cigarettes for a long time.. his oxygen saturation is 84% (normal is 95-98).. the pts inability to maintain adequate tissue oxygenation will most likely lead to increased production of which of the following in his erythrocytes?

I narrowed it down to 2 answer choices..

- 2,3-BPG (the correct answer)
- EPO (my answer)

I see why 2,3-BPG is correct, but I do not understand why it wouldn't be EPO.. Isn't the pt hypoxic? I thought hypoxia leads to EPO production.. I am missing a very simple concept here and would love someone to clarify for me, thanks!

the question is asking about the erythrocytes, and EPO is produced in the kidney man.
