Default ECFMG application

Hi guys, a need you advice. I'm IMG.
I have completed on-line part of application about 6 weeks ago and sent my 186, 345 form 2 photocopies of my medical diploma etc.
3 week ago my Certification of Identification Form was accepted and now it is valid.
Yesterday I received an e-mail that states following:
'...the degree title you indicated is not the medical degree issued to graduates of medical schools in the country where you are attending medical school... To be registered for Step 1 , you must reapply by submitting a new online application completed in its entirety.'
I found out my mistake and at the end of my current application I have 3 variants:
1. I have graduated from medical school and have previously submitted to ECFMG photocopies of my medical diploma.
2.I have graduated from medical school. I have a valid Certification of Identification Form (Form 186) on file, and I am submitting two copies of the Medical School Release Request (Form 345); two photocopies of my medical diploma with an English translation (if required); and a photograph.
3.I have graduated from medical school, but my medical diploma has not been issued. I have a valid Certification of Identification Form (Form 186) on file, and I am submitting two copies of the Medical School Release Request (Form 345); a letter from my medical school that confirms I graduated, have met all requirements to receive my medical diploma, and states the date my medical diploma will be issued (refer to Eligibility for Examination in the ECFMG Information Booklet for the exact language required in this letter); an English translation of the letter (if required); and a photograph.

I don't know what variant to pick and if I should send anything to ECFMG by post. Or just this on-line form is requested.

And one more: My name in passport an in ECFMG is 'Semenistyy Anton" but name in my medical diploma is 'Semenistyy Anton Alekseevich'. Is it a problem?
My medical degree in my diploma transcript (that was sent with my form 186 and as think was accepted) is 'Medical doctor' but according to Reference Guide for Medical Education Credentials and my current on-line application it should be "Doctor of Medicine". Is it a problem or it is enough that changed it in my on-line application?

hi anton /......ya the name is definately a problem u need to provide a letter abt ur spelling on name ...u can call ecfmg people directly they will tell u the procedure ...i had same issue i have sent them two forms of 186 ...u can jus call them and ask which form do u need to send ..they will explain u clearly

Thank you for response. Are you sure that it is a problem? "Alekaeevich" is just a middle name... Is it necessary to start new application or I just should send them a form with clarification?
