Skin Miss Pepita Forehead Lesion!

Miss Pepita goes to see the family doctor because of a skin lesion. After clinical inspection, she was found to have a skin lesion on her forehead that is related to hyperplasia of the stratum corneum as well as retention of cellular nuclei in the stratum corneum (See image below). Histologically, this lesion is confined to the epidermis.

According to the image and the description above given. What is the most likely diagnosis?
  1. Lichen planus
  2. Acanthosis nigricans
  3. Cutaneous horn
  4. Erythema nodosum
  5. Pityriasis rosea
  6. Squamous cell carcinoma

I know it might sound not very tolerant for a doctor, but I would really like to know, why wouldn't she cut that thing off?

I mean she obviously wears a shirt and calmed her hair, so she sortof tries to look normal. Yeah, I know she is not 17 anymore, so I am not even thinking about her putting on makeup or something like that, but... why wouldn't you cut that thing off????

Is there any purpose behind it?
Like you can hang a bag on it while doing groceries so you can have free hands...
Or maybe its Elvis Presley tribute? Like a part of that funky hairstyle, 50-ties badass?

Do you think its possible that there are an underground unicorn-mosh-pit fightings going on and the modern society is not aware of it? And this lady is like a lifetime champion in those grotesque fights?
If yes - I would bet money on it!

I am seriously bothered by this picture...
I apologize for being intolerant, but why wouldn't she cut that damn horn off?!

I feel you there man... That is actually the main reason why I posted this... because I don't understand... I can't understand... and I think that I could never really be able to understand why. I searched for the answer... there was none. And then I though of you guys... and wanted to share my with the advantage of learning something out of this.
