Smile DIT in 21 days cont'd

Good morning!
Continuing with DIT in 21 days, yesterday I finished day 8 videos, 7 more days to go. If I do 3 videos every day I should be done by next monday i.e 4/23/12.
Starting day 9 vid 1.

how about questions, are you getting any done along with the videos?
I'm on day 10 and i just cant do any questions along with the vids!

my scores since DIT:
52, 58, 58, 58, 65, 58, 65, 69

prior to DIT, i would say my average was 55%
from these scores, its now 60%

so i guess its gone up 5% during the 9 days of DIT ive watched.

my scores in UW have also risen

i just dont get how this guy does it! i mean all he's doing is reading to us from a book! and still, every couple of days i feel like im on a higher level than i was before!

because when u actually use ur hand write the answers to questions.. U must reach the point where u say to urself... "looks like somebody hasnt been studying with his heart, thinks he's studying for a medical school exam where i can just memorize the last three letters of that drugs name and then i'll be able to answer the MCQ on the test''.. But not for step1 baby
Only hard work will pay off, my advice is study with full conscience and no cheating, after finishing three videos study the material like if the test was tomorrow... Even if takes 40 days, not just 21, its more important than solving questions.. Coz u only have to do this once, do it good,,,, and after that, a full thorough read of FA can be done in less than a week if the DIT course is given its right dedication and detirmination

Finished 1st vid of day 9....I had to run an errand.

@rennaisance...I am doing 15-25 qs UW every day .And the in quizzes he gives after the end of the day, I score 99% every time...I think thats his key. If we listen to his lecture keenly( its audio-visual), its sits in brain better than when we study by ourselves + the Right brain bonusses little skits are great + the quizzes re-inforce the material. I can't wait to do some NBMEs after I finish this course.

@Dr.nick...u went from 65 to 69% in less than a week...amazing & your steady increase is awesome...keep it up.

@Hopetopass...Iam enjoying the course & am sure you will too... but at some points it gets tedious...just have to keep going.

im really disapointed with how he covered the lysosomal storage diseases, he didnt review the text at all, just threw us into a quiz of it and then moved on.
first time in 10 days that i am disapointed in DIT.

How do know all these people are not working hard, not studying with full conscience, and are cheating ( btw cheating who?). Some of us are studying for 10-14 hours /day, and revising everything at the end of 3 lectures- I don't know if you heard dr.jenkins right, he says to solve qs along with his course, thats why all these people are applying their knowledge and raising their scores, nothing wrong with it..... then why are you saying..."Even if takes 40 days, not just 21, its more important than solving questions.. "

im starting day 3, video #2 now
tomorrow [friday and saturday] is my weekend, im tearing that thing apart, hope to do days 3 [4,5,6] during the weekend, and finish day#3 today
You are advising us to do it in 40 days yet you want to do 3 days lectures in 2 days.

done with day 10 vids. He brushed through many parts of genetics, had to read thru kaplan LN (which I read long time ago) to understand concepts.
He explained gel electrophoresis well, & yeah lysosomal storage dz- he didn't even read a word from FA...directly went to quiz, but he said it is not HY ( which I didnt expect)...saves me a lot of trouble...I remember spending a lot of time studying it...practising the enzymes & products pathway, now I'm going to brush thru the main differentiating points. Exhausted..I have been hanging with these books for 14 hrs now....time to call it a night. GN

Finasteride- rx of BPH, male pattern balding
Flutamide- only in rx of prostrate cancer.
Ketoconazole & Spironolactone-
- used in rx of PCOS to prevent hirsutism.
- They have SEs of Amenorrhea & gynaecomastia---why? is it b'coz of dec testosterone---> inc estrogen?

they block androgens (test and estrogen) at their receptors so they cant perform their actions
estrogen cant perform its action on the endometrium, so amenorrhea
testosterone not performing its action in men = gyno

@nick..I didn't see your explanation before, thanks....makes sense. may be I missed it, where did it say they act on receptors, I dint know that, only knew that they inhibit steroid synthesis & binding.
How is your prep coming along.

starting day11vid1 now
scored 60% in UW today, not great but still in the 60s, which is where i want to consistently be right now.

@pathophysio1..... my plan is to take by june 5th...hopefully will be ready & not postpone it.

@nick....60s consistently is very good...in next few weeks, maybe u'll jump to 70s....GL. I am planning to subscribe(2nd time) to UW as soon as I finish DIT ( 4-5 more days) and will start doing UW + nbmes + FA revision.

On to 2nd vid of the day.

How did you study cellular biochem....really bogging me down. Just studied those CDKs & cyclins from DIT notes...how imp are they? It's all pure memorization.

i think that CDK stuff he talked about today is overkill
thats too indepth
that wont be on step1

no other source goes into that much detail of describing every type of CDK.

just know what a CDK is
and know all the apoptosis pathways and cell cycle stuff and its enough

p53, bax, bcl-2, cyclin D, etc is what u need to know.

oh man its one of those translocations, umm Burkitts? chr8 = bcl-1 = cyclin D?

or was it bcr = cyclin D?

is it multiple myeloma?

I was doing research earlier on google while studying cyclins & CDKs ...

one source says "Cyclin D1, D2, or D3 expression appears to be increased and/or dysregulated in virtually all MM tumors despite their low proliferative capacity."
Other sources say leukemias & lymphomas ( so its ass w/ cancers- blood mostly). If u find out something..let me know.

This cellular stuff is complex Iam going to now just stick to FA & DIT highlights on this cellular biology--already wasted lot off time on it.

****, bcl-1 is in mantle, not burkitts

so the answer is Mantle cell lymphoma? (had to check FA to see where bcl-1 is but i do recall from kaplan bcl-1 is linked to cyclin D... i think lol)

Yup, The answer is Mantle cell lymphoma; t(11:14). Chr 11 has bcl-1 (Cyclin D) which gets translocated to Chr 14 (IgG heavy chain - which is always active)...This results in Cyclin D hyperactivity


In Burkitts, its t(14:18) where Chr 18 is bcl-2 which gets translocated to Chr 14 (IgG heavy chain) and leads to bcl-2 activation which then inhibits apoptosis by blocking the bax channel.


Whats the mechanism of Cyclin D? Its an oncogene that phosphrylates RB gene leading to its activation which causes cells to move from G1 to S phase?

Ah yes you are right. Sorry, my brain was still hung up on Burkitts coz I had just read about it! This is follicular lymphoma.

Friends, keep the qs coming...best way to keep us challenged on this cellular stuff. Not too difficult ones though( Phd level)

couldn't sleep well last night, had to take anti-histamine at 4 am , woke up @ 11.30. I was doing good waking up early lately....I soo dont want become a night owl again.

Also, if there is a mutation in Laminin's, wouldn't it restrict any cancer from spreading since those are used by cancers to spread from cell to cell ?

@nick...couple of months ago I saw a documentary...on HBO or TLC...cant remember....very interesting....2 extreme opposite cases- one 5 y/o kid looking like a 60 y/o man due to accelerated growth ( Progeria) & another case of a 40 y/o man looking like a 6 y/o kid due to slowing of growth process( I forget the disease name...when i have time, i'll research it).

Its cases like these that keep my interest in medicine...not studying for step 1....cant wait for step 1 to be gone out of my life so can be excited about real cases....I know...I Know..need the basics too.

finished day 11 vid 3. Thalamus & hypothalamus, he went straight to quiz.Cerebellum was covered well. Had to go out to the bank to notarise some forms- lost 2 hrs.

I am doing DIT now.. I am still at day 8
but I was wondering, is it good to take it for a 2nd time one month before the exam??, or shall I just keep reading FA and Qbank?? .. well it add alot to my performance??

Doing before the exam will definetely keep it fresh in your memory... it is time consuming though.

I felt exactly the same today, neuro got to me......i did only only half hour of video and couldn't take it any more & was feeling down.....so my family dragged me out.....went to a restaurant, did some shopping and had a good time. I feel refreshed & am going to pull an all nighter & finish neuro.

Just take some time out & do something you enjoy...u'll feel a lot better, can't let usmle blues affect us.

Not liking DIT neuro at all...I'm pausing & studying by myself & playing it just to listen him read fast thru each diseade/pathway/lesion.....taking a long time.Atleast the quizzes are helpful. But I'm determined to finish day 12 videos today.

drj1, when're you planning to take the exam? What other resources have you used besides DIT? I'd like to start DIT, too, but I am brushing up on my basics first from some review books. Would like to take the exam end of June. Finished only about 200 questions from Uworld. Do you think it will be easier for me to go through the DIT videos since I will have finished reviewing everything in FA from other resources first? I am also annotating as I go along. You're making good progress. Keep it up.

I did kaplan LN & videos once ( its been a while since I did them ), I did UW once, its been 2 months since my subsription expired, planning to take it again after finishing DIT. I gave one complete reading of FA cover to cover once & again 2nd time covered more than 50 % of it, then decided to do DIT....its been a long & tiring process. When I started DIT 2 weeks ago, my energy levels were up, now as I am coming to the end I am getting exhausted.....my advice is take regular breaks...I only took 1.5 days off so far, hence the exhaustion....3 more days of lectures to go, I have to find the strength to finish it.
Definetely do DIT...it'll help cement what you've learnt so far. Do UW qs along with it if you can. GL

I'd just like to ask... Are you doing DIT with a qbank right now? I thought you were doing uworld but from your post here, it looks like I misunderstood... Are you using another qbank? Why not do UW second pass with DIT? Sorry for the many questions...

I'm asking bec. I'm considering doing my 2nd pass of UW with DIT. It seems that strategy will save me time -- like two birds with one stone. But I'm torn if I should do combine DIT with an unused set of questions from kaplan qbank or usmleconsult qbank instead. I'd like to hear your opinion on it. Thanks!

Since doing DIT is towards your end of your prep, where all you focus on are HY material, i think the combo effect of DIT + FA + UW will be the most important to focus on.

Dont know, just saying -second pass of UW along with DIT sounds better!

I'm finally squeezing in 25 Qs per day of UW along with the DIT day.

Hopefully i can finish DIT and UW together, so i can take an NBME and see where i am.
Very scary, since i failed my last NBME around 3 weeks ago -but i dont know i feel way more stronger than my last time.

I am doing UW offline version ( not many ..around 25/day ). Its hard to do many qs b`coz most days I'm doing 3 lectures a day. The offline version I have is not in random blocks, it is subject wise so I don't get the exam feel while doing them. When you said 2nd pass UW...is it online version.
Doing DIT with qs that give you an exam feel( unused random block) is the best way. GL

sorry guy I missed all your comments, I have now officially bacome a night owl. I was awake all night yesterday..slept at 5 am & woke up now @ 2.30 pm. I'll finish this DIT & reverse my cycle back.

@rennaisance...yes, my limit is 25 qs too on DIT days. I havent done any nbme so far......postponing it DIT. GL on your next nbme.

just finished 1st vid of the day. I had done respi system from FA recently so this DIT respi is going fast. MY advice is read the correspondig FA chapter well before you do that chapter DIT lecture...it'll go faster.

I haven't scheduled my exam yet. I'm planning it on the last week of May or some time in June. I'm already in my extended eligibility period.

That's what I'm thinking too. Thank uuu!!

I don't know where to fit UW 2nd pass in my sked if I don't combine it with DIT. I don't want to prolong the step 1 agony anymore by giving UW 2nd pass an additional 2 weeks.

@Nick & @dwiqed - Did you guys take your NBME yet as you had planned ? When are you guys taking your NBME ? I am looking to take my NBME around Thursday or Friday next week (26th or 27th). And after my NBME then I will start DIT most likely...All of May - Qbank + DIT

I finished uworld with a cumulative score of 65%. But it's not accurate. I did it subject-wise, timed, small blocks.

Yep! I took nbme 5 (online) already. I'm currently at 228. It was a big relief! Not much but it's a good start I guess.

I'll do a complete run of UW notes + FA + Goljan audio for the next two weeks. Hopefully, I can squeeze in Goljan audios... It's my first time to listen to it. (I just sampled an hour of his lectures before.)

Then, I'll take nbme 12.

After that, DIT + qbank on the first week of May. Hey, if our skeds go according to plan, maybe we can be DIT buddies!

Hey friends, 2 qs to all of you...
1. After finishing DIT in 2-3 days, I want to revise FA quickly in 3 days and take my 1st nbme on 28th and couple of nbmes after that. so which nbme should I take 1st which ones later...is there any order to follow.

2. Should I continue with offline UW or should I buy a new online subscription.
Please advise. (just reminding I'm planning to shoot for june 5th exam date if all goes well ).Thanks.

Nice! Good plan. I hope I am above 200 when I take the NBME next week. I am scared to take it I will take NBME 5 as well. For the next two weeks I am just planning on working on my weak areas, do questions, take the NBME and read 100 cases of Medical Ethics. I might add in UW notes that I have and go over them as well.

Sounds good! Yeah I am planning on starting DIT along with Qbank in the first week of May as well. That would be nice, we can keep each other motivated!

Take an online NBME. It depends on how many NBME's you want to do in total. The best ones to do are 7, 11, 12, and 13. But if you want to do more then you can do 5 and 6 as well. Save forms 7 and 13 for the last NBME's that you do.

And you still have about a month and a half till your exam so its always good to keep doing questions. May be extend your subscription.

wow! 228 and u havent even done DIT yet!?
i guess you're shooting for 250+ eh

im taking nbme form 12 (online of course) on 27th or 28th.

@HopetoPass and Drj10:

If you guys haven't done them already, try the offline nbmes first (forms 1-4). I did all 4 before nbme 5. My scores clustered around 228. If your scores cluster, more likely they will be predictive of your online nbme. If not, maybe some forms hit your weaknesses and the others hit your strengths. You can work on your weaknesses for a day or two then give an online nbme. Doing them will give you confidence that online nbmes may not be that scary as we thought.

@Drj10: HopetoPass is right. If you plan to take only a few nbmes, you can choose from the newer ones (forms 7, 11, 12, 13). On the other hand, if you just want to see where you are in your prep and you don't mind spending a bit more, you can start with nbme 5. They say it's the easiest. It's good to start with it if you're having nbme jitters (like me). But if you're confident about your prep already, then start with nbme 12.

Nbme 7 and 13 are considered the toughest. Take them when you're sure you're going to get your target score already.

I worked hard on my weak subjects in uw. I even watched kaplan videos...

I watched them in bed for a couple of days. I swear I thought I was having decubitus ulcers already...

im barely getting through 1 video a day

my left eye is acting weird so i have double vision and its making it very hard to read and concentrate

Toxicity of DIT:
extreme exhaustion,flight response from all things usmle, eye twitching, diplopia, impaired comprehension & focus, anhedonia( loss of interest), neck spasms, low back pain, in extreme cases- depression, what else?

What is the rx of this toxicity?

I am almost avoiding DIT today, doing some qs, hopefully will get back into groove by this evening & get 1 video done.:sorry:
