USMLEWorld Prussian blue stain (UW question)

From UW Q id 185

- - - - - - - - -

How come the patient had "CHF" and not interstitial lung fibrosis????

No detailed history was given or whatsoever (just that the old guy died )... but for me, interstitial lung fibrosis is also a valid and a possible answer.

Asbestosis, which is a pneumoconiosis (Which is also a type of interstitial lung disease), has also golden intracytoplasmic inclusions that are blue with Prussian blue staining (also within macrophages).

What I'm missing here???
"Disease is very old, and nothing about it has changed. It is we who change as we learn to recognize what was formerly imperceptible." JMC
