Yes, it will. A failure at any point stands out like a sore thumb. This doesn't mean the end of the road though. You'll just have to score that much better on your USMLE steps.
Also try getting a decent score in your third and fourth years of MBBS. I am sure some of the PDs will appreciate a candidate who learns from their failures and goes on to do better.
