There is alot of information which you can find in the forum by using the search option.

Generally speaking you need FA (First aid 2012).
You need to register to qBanks i recommaned first doing kaplan and then Usmle world.

there are 2 main ways of studying to the usmle.
using kaplan notes (KLN) for every subject + videos.

or using different book for each subject.
Because i choose to do the latter one my recommandation are kaplan vidoes for all the subjects.

Behavioral science - Kaplan videos + FA + Ethics (100 cases by fischer)
Physiology - Physiology by Linda costanzo
Pathology - BRS pathology + goljan audio / you can use instead the rapid review pathology.
Pharmacology - lange katzung and trevor
microbiology - videos, FA , ridiculous microbiology

while doing the qBanks you will learn alot of new things.
Later on in your studies you can do NBME assessment tests which predict your score - 1 months before exam, 2 weeks before etc.

Good luck.

It's very individual.
it mainly depends on your basal knowledge. I am a medical student so most of the information is still fresh in my mind so for me 3-4 months were enough. For postgraduates usually 6-10 and more months are used.
