Drug The Drug of Choice for CMV Infection

2 Months after kidney transplantation a 37 year old man has been put on immunosupressive therapy. He developed cytomegalovirus infection. Which of the following is the most appropriate therapy?

A- Acyclovir
B- Ganciclovir
C- Famciclovir
D- Ribavarin
E- Saquinavir

Star An extra point

Here's an important point:

Treatment of CMV and pneumocystis jiroveci

Never use Ganciclovir with TMP-SMX because they both cause bone marrow suppression, therefore better use Ganciclovir with Pentamidine.

Never use Forscarnet with Pentamidine because they both nephrotoxic, therefore better use Forscarnet with TMP-SMX

Warning! Valganciclovir

According to the latest Kaplan CK notes, the drug of choice for CMV is valganciclovir because it can be given orally and IV ganciclovir can be reserved for severe cases only.
Second line choices are Foscarnet and cidofovir
