Help Nbme q pls check

Pls can some check nbme form 5 section 2 Q number 45 and tell me what the picture is.
Another one is NBME form 5 section 3 Q number 1 . pls how did they arrive at their ans.

Form 5, Section 2, Q45
This is Hands Foot Mouth Disease
It's caused by Coxsakie A which is a picorna virus which is a single stranded RNA icosahedral with no envelop

doesn't the NBMEs change up their questions and blocks. as an example- what was block 3 for 1 person is now block 2 for someone else and the questions are moved around. So unless you have the picture you can't answer this, can you?

No the NBME does not change the blocks! It's always the same four blocks in the same sequence for all the forms.
Maybe you are confusing with the USMLE exam itself, then in the exam the sequence of blocks and even the questions themselves vary from one test taker to another even if they are siting in the same exam center.
