Arrow NBME 7.. need a few answers...

block 2
Q28 - why is it germline mutation? her mother has it too right??

Q39 - if we give Angiotensin for an hour... wudnt the blood flow decrease..??

block 3
47 - i marked D, but it is wrong...i thought moderate levels of anxiety are good for the pt and KLN mentions that risk of non adherence should be explained for more compliance... what is the answer... can anybody explain...

block 4
Q14 - have no idea... its a negative log... but what does that mean... apparently potentiator is not the answer...

anyone done with the NBME... could you please help me with these...

n0.28---- i have the same confusion.


4----14:

potentiator is supposed to decrease the dose conc. as far as i remem the last curve had more conc to achieve the same efficacy. so comp antagonist.

Q39

AGII is used in low-volume states (decreased renal blood flow) to increase the filtration fraction (increased FF) by increasing the GFR (increased GFR).

Hence, i think the more correct answer is A, increased FF.

Q14

There's a silly mnemonic i came up with to help me regurgitate this concept in a split second (no time to think during the exam)
CPR = Competitive (Km) - Potency - Right shift
NED = Noncompetitive (Vmax) - Efficacy - Down shift

Of course you know which ones decrease and increase
-------------------------------------------------------

I just did it a couple of days ago, got a 214

wow!! you are done with CS and step 1?? congratulations. I think 214 is taking you to a fighting position. good luck !!!

I just did it a couple of days ago, got a 214
39 - angiotensin does increase filtration fraction but giving it for an hour ... i thought due to efferent arteriole constriction renal blood flow will eventually decrease..k.. can u tell how the coefficient of filtration will change... i dont get that...

14 - hey that is helpful... things like these are pretty confusing in the exam especially when ur mind starts playing games with you... thanks..
and hey dont worry... its not a bad score... go over ur weak areas... u will be jus fine

These are useful information and I got the concept ,but in the nbme 7 q there r two sustances on both sides and the ask how we know the direction of the reaction in case u know the energy on both sides/?

wats the answer?? wats the diagnosis? i thought its pumlonary edema due to CHF .. so BNP/ANP would be released leading to increased GFR??
key says D.increased k+ excretion? please explain

no 50 .. how do u get to the answer??? i know abt the delta G value being negative n positive and what they mean and all but how the hell do you solve THIS one????

please help
